This Privacy Policy is incorporated into our Terms of Use. By using our Services, you are consenting to the practices described in this Privacy Policy and you agree that your personal information will be handled as described in this Privacy Policy. Scope of this Privacy Policy and Protected Health Information
This Privacy Policy describes the types of Personal Information we collect from visitors to our website and who utilize our Services, as well as our practices for using, maintaining, sharing, and protecting it. It also describes the rights and choices you may have with respect to your Personal Information and how we can be contacted.
This Privacy Policy does not apply to information collected from you if you register and log-in to the password-protected and secure portions of our Services, which are intended to be used by potential patients of independent healthcare providers for the provision of their respective professional services (the "Platform"). The Platform allows eligible patients to use certain Services related to the provision of the practice of medicine and other healthcare related professions. All information collected and stored by us or disclosed by you within the Platform is considered Protected Health Information ("PHI") and/or personal data and shall be governed by applicable state and federal laws that apply to such information, including the Health Insurance Portability and Accountability Act ("HIPAA"). The Notice of Privacy Practices (and not this Privacy Policy) also applies to Personal Information collected through our Services if specific symptoms or health conditions are also collected, as such information may be considered PHI under HIPAA.

- If we decline to take action regarding your request, we must notify you without undue delay, but within forty-five (45) days of receipt of the notice and provide information on our appeal process. We are required to provide justification and provide information on the appeal process within a reasonable time after the communication of the decision to decline.
- You can request information free of charge twice per year. For a third or subsequent request within a 12-month period, depending on your state of residence, we reserve the right to charge a fee for our provision of your information.
